• Create a new transparent layer on the bottom
• Choose the custom shape tool
• Set the mode to fill by clicking the icon that looks like a square with handles on the corners. The icon is located in the Options Bar at the top of the canvas.
• Select the Heart shape from the Shape field pull down list in the Options Bar
• Draw the Heart and position it
• Put your desired photo on a layer above the shape
• In the layers palette, right click on the photo layer and select Create Clipping Mask
• File > Save for Web and Devices
• Choose PNG

Turn the background into a proper layer.
Use the custom shape tool to draw the heart onto the picture.
Select the part outside the heart, then delete it.
Save it as a GIF or something else other than jpg, since jpgs don't support transparencies.
If you're ok to have the outside of the heartshape in white, then jpg is fine.
You might find some of those terms confusing, but that can't be helped. Photoshop is a HUGE program and you will need to spend many, MANY hours learning, reading and experimenting.
It is a big effort, but very much worth it. Good luck.
