It depends on what state you live in, because laws are different from state to state. i would call your local Social Security Office and ask them. They should be able to give you all the details.

Request new birth certificates with a name change, passports, and then take all of that to the school board so they can change their names on the school records as well.

You need to get a new birth certificate and legally change the name.

it all depends on the reason for the name change. the procedure would be different if its say an adoption or if a paternity affidavit was recently signed etc

I believe you first have to petition the court. Call your courthouse and ask.
