Usually a meat or poultry item. Try having that macaroni goodness with meatloaf, a burger, steak, hot dogs, pulled pork, any other preparations of pork, ribs, lamb, a deli sandwich with any kind of deli meats, sausages, chicken and turkey prepared in various ways.
If you like fish, strangely enough fishsticks work well with mac and cheese.
Ok, now I'm officially hungry and I LOOOVVVEEE macaroni and cheese!

Vegetables! Any green vegetable you like.
Roasted vegetables:
Line a cookie sheet with aluminum foil.
Preheat oven to 400 degrees. May use temperature you are using to bake the mac and cheese but will take a little longer than the usual 20 - 30 minutes.
Wash vegetables, trim when necessary. Try brussel sprouts which are wonderful or asparagus.
toss with 2 tablespoons olive oil and spread on the cookie sheet. Place in oven. Roast until begin to brown. Roasting really develops the flavor of the vegetable. You can mix in other vegetables such as red bell pepper in large dice, cubed butternut squash, red onions, add cloves of garlic.
You might serve more than one vegetable at a meal or else add a green salad.
Fresh fruit for dessert!

I'm in college so I make mac and cheese (shells and cheese :) mmmm...) all the time! I pair it with flaky, buttery biscuits and a canned veggie like green beans.
