How do I word a release for clients to print my photography?

I am a part-time photographer. I do not print the photos, but rather save the client money by giving them a CD with the finished pictures, ready to print wherever they choose. Do I need to sign a paper saying that I give them permission and if so, how would I word it?

    Usage License.

    I (photographer) give permission to (client) to make physical prints from the images I provided on the CD entitled _________________ (your signature on which matches signature at end of this license)

    I being the photographer and only copyright holder of these images am entitled to provide such license.

    Name printed, name signed, date. Maybe give a header with your name and phone number.

    You want to have some way of identifying that the photos you give on CD are the ones that go with the license you give. You can list the numbers, include thumbnails on your license, identify the CD title which you have signed, etc.

    One suggestion is to go to where ever people in your area are likely to take a CD to have prints made and ask them if they have a preferred license agreement and/or if the one you have is adequate.

    You can do a search online and learn for yourself what you need. You will become very familiar with laws (especially in the area in which you live) if you do this yourself. Protecting yourself from any possible lawsuit. Doing the research yourself will limit any possible damages from someone directing you to a specific website with something that is false, or very poorly written.
