You owe the hospital money. You have debt to them. The hospital "owns" that debt.
A debt is an asset. It is a "promise to pay money". Your promise. Debt can be sold. Someone else could buy your $5000 debt (they would pay less than $5000, because they would be gambling that you would pay). If I bought your debt then you owe me INSTEAD of the hospital. If they sell the debt, you no longer owe it to them.
You can only owe the debt once.
Two different things can happen with collection agencies.
#1, the hospital can hire collection agencies to HELP collect a debt the hospital itself owns. They can hire as many collection agencies as they want.
#2, the hospital can SELL your debt to the collection agency. they can only sell it to ONE agency. (or, if you owed $5000, they could sell $4000 to one agency and $1000 to another agency.)
What you have here is case #1. The hospital still owns your debt, but they have hired several agencies to collect it.
What is the WORST thing that can happen? Suppose you owe $5000. You pay one agency the full $5000, meanwhile your wife pays the other agency the full $5000. Ouch. What happens then? All the money goes back to the hospital. The hospital takes $5000 to settle your debt and then credits your account the other $5000. This is YOUR MONEY and you can ask for it back.

Pay the bill seems like a definite possibility.

If you still owe the money, of COURSE it's "legal".....that's a bit like asking if it's "legal" for more than one cop to chase you as you leave the scene of a crime!
