When writing a Japanese name, should we use space to separate the surname and the given name?

Example:The person name is Suzuki Kentarou in romaji, should we write it like

鈴木健太郎 or 鈴木 健太郎?

Please this makes me confused, I'll appreciate you if you help me...

    Formally, no space like 鈴木健太郎.

    However, in forms such as application form and etc., space is often inserted to make it easier to differentiate the surname form the given name.
