Dental is always seperate, and I haven't seen any dental plans that pay out more than they cost.
For health insurance, you get what you pay for. If you're perfectly healthy, male, 20, for $100 a month, you're looking at a $500 to $1,000 deductible on a decent plan. That means, you pay the first $500 to $1,000 of medical bills, before the insurance kicks in.
If you're older, female, or not perfectly healthy (including overweight), it's quite possible that you won't find ANY type of "insurance" for under $80 a month.

It depends on how old you are, but just keep jacking up the deductible until you get down to that price. There are a handful of companies that would be able to do that. You just need to find an insurance broker that does that there. In the interim I have comparison tools and information on my site that would help.

You have to call an independent insurance agent where you live who can give you some quotes for insurance. I would not rely on anyone on YA even if they lived in Nevada. We dont know what companies are there.
