Could someone translate 2 little words to French for me?

I'm making a gift tag for a friend's birthday present and I need to know the correct forms of the words 'to' and 'from'. Ridiculously simple, I know, but French grammar baffles me. Thanks in advance.

To can be à/au/aux/pour/de/des....So I'd recommend looking it up on this site http://www.wordreference.com/enfr/to

From is usually 'de' but 'de + le = du/de +les = des'
