What formulas and methods do they use to find out what the approval rating is and how often? Who rights down these polls and are they biased?
How is it determined what the "approval rating" is for the president? And how accurate and reliable is using a politicians approval rating to judge whether people agree with him or not when so many people never got to vote in it?

Many polling companies, such as Gallup, Rasmussen, and Quinnipiac conduct national telephone polls. They randomly call people throughout the country (or region depending on the poll). Because there are so many people in the country it's not surprising that you have not received a call yet but you may in the future. Also, you may be able to sign up for their services to vote more often.

It's accurate enough so that both liberal (MSNBC, C-Span, CNN, NBC, The NY Times, The Huffington Post) and conservative (Fox News) media use it to report statistics.
Alawex has a very good answer. He was more specific and provided the methods employed by the companies. Give him best answer, he cares about it. I just go around arguing with people instead. ;)
