What if any information is absolutely needed for a successful inquiry via a dd 214 form ?
This inquirey was made post the recommendation of a fellow Yahoovian that I file a dd 214 form, which for all I know could be a requisition for a skyhook to hold my extention cord cord / work light ...
But be that as it may, I seem to recall I was issued a military ID number, separate and distinct from my Social Security number; is this memory accurate, and or do I need this? Would my Social Security number be the serial number noted by answering parties or at least be sufficient? Given my degree of injury, I have developed some degree of problem in keeping numbers in their proper order; if I put the numbers in the wrong order, will I be laughed out of the room, as it were?
Are there dedicated personel who help the disabled military veteran to help get one past the clerics?

Go to the www.va.gov website
Apply over the website...
Fill out form SF 180 requesting your DD 214...
Make a"Hard Copy", "Sign it and Mail It...
If you were in the Service before May 1968...
You have a Separate Service Number...
After May 1968 you used your Social Security Number...

Name, rank and Serial number. Dates of service would also be useful but probably not absolutely necessary. Good luck to you

Depending on when you served, hence where your records are stored, you would order your records from that location.
Location of Military Service Records, Official Military Personnel Files (OMPFs) and Health and Medical Records

Click on your service from the menu on the right under;
"Locations of Service Branch Records".
