Sorry I have so little information, but I have been warned it is a bureaucratic nightmare from the very pits of hell. The FDA is considering mandating that all forms contain a warning label informing users that filling out the forms will cause severe, irreversible brain damage. The process is described as the ultimate time and energy vampire. Not trying to discourage you, but keep and open mind about insurance fire.
