The Scarlet Letter is a book by Nathaniel Hawthorne. It is about a woman who has sexual relations outside of marriage during Puritan American times. Her punishment, instead of the death that should have been her punishment, was her having to wear a scarlet "A" (which stands for adulteresss) at all times. This way she would always be reminded of her sin and would be shunned by society.

So the line from the song is basically her saying she snuck out to have sex with "Romeo"
