You will not get all those out even if you distil the water or use reverse osmosis! ;-)
I like Clear2O pitchers, but the replacement filters are too expensive. Then there is Britta, cheap but I read some reviews that they do not always deal with the chlorine and the charcoal tends to leak out. I went for PUR 2 stage - the filter prices are reasonable and they had better user reviews.

Wow, that is quite the list to filter.
Generally, There are 2 standards to which pitcher filters filter to: NSF/ANSI 42 and NSF/ANSI 53
NSF 42 will remove:
Hydrogen Sulfide
Chlorine
Chloramine
Particulates
NSF 53 will remove:
Lead
Chromium
Mercury
Anything that you want to filter beyond this will require a water filtration system. Reverse Osmosis and Distillation are two of the most comprehensive water filtration methods. See a summary of all of the types of water filtration at the link under sources.

There isn't a pitcher filter on earth that will remove all of the contaminants you listed. If you really need these contaminants removed, a reverse osmosis system http://www.waterfilters.net/PuroTwist-4000-Reverse... will be your best option.
Having said that, are you certain that you actually have these contaminants in your water. I'm all about high quality pure water, but it is extremely unlikely that your water actual contains all or even many of these contaminants.
If you are as worried about your water as your post seems to suggest, then you really need to obtain a laboratory analysis of your water. http://www.waterfilters.net/Professional-Laborator... This will provide you with the information you need to make a decision based on the facts.

There's actually almost no difference overall. Of course, you want to make sure there aren't too many microorganisms in it that could infect you, and you may not want the caffeine in coffee, or the sugar in soft drinks, and you may live where water is softened and has enough sodium in it to be a problem for people with high blood pressure, but overall, it's all water, and your body will use the water the same. What else is in the water may or may not be important to you.

geebus! is there really all of that in water? The best For the money i have found is brita. plain and simple. you don't have to attach it to anything, and you just pour water in, and change out the filter once in a great while. you could buy them off ebay too, reducing the price even further. great to keep you in shape, and keep your costs down.

Dr Oz had a show on it last month about city/well water and filters. Search Dr Oz's website to see if it is still on there.
