Any unique birthday gift suggestions?

Question

OK so, my best friend's birthday is coming up later this month.  I'm trying to top the best friends picture frame hanging jewelry box she had gifted me and the clear friends picture frame she bought my other friend for her birthday.  Now, I have already gotten her jewelry and some bath & body works lotion.  Any suggestions that are unique and interesting that could top her gift for me?

The most interesting and unique gift is one that only you two would ever understand (: For example... one of my best guy friend's birthdays came up, and we had a hilarious inside joke since he always kept hurting himself, I threatened to wrap him up in bubble wrap, and in the end, for his birthday I actually did get him rolls of bubble wrap and he loved it!
