A massive card with 19 and little sybols of things she likes eg. Cras, favourite band, shoes/clothes etc.

Since you've been best friends since you were 2, why not make her a scrapbook :)? Fill it with pictures & other things that remind you of her. I'm pretty sure you've gotta have pictures of you & her!
