12 in 4 years doesn't seem that high a number to me. It's a tragedy yes, but not suspicious.

There are probably lots of answers, but we'd need data to reach any conclusions.
Is there industrial or other pollution? It hits infants and the elderly harder than the rest of us.
Are the parents younger than average? Lack of parenting skills correlates with infant mortality. "I thought it was safe to..." when it wasn't.
Are the parents likely to be uninsured, so they don't get routine medical care for their babies, or advice on their well-being? That leads to higher mortality.
Are delays of several days the norm when parents call for a sick-infant appointment? Overtaxed staff means infants get sicker before being seen, and the sicker the infant, the higher the mortality rate, of course.
In the end, we can't know why without further information, just know that it's a tragedy which may well be avoidable.
