I've been seeing it a lot as well. It's mostly on YouTube, but sometimes on other web pages. Seeing as there's none on Yahoo!, I think it's just Google Adsense ads. Google must be going crazy right now trying to figure out what the problem it. I have an adsense account myself, and have noticed a slight decrease in revenue since it started (not much though).
But yeah it's definitely not your computer. Consider it a good thing, less ads means faster internet and less CPU usage.

it's a data base error on the ad server - 404 = not found Or an ad blocker

The website has been deleted or it's a web server problem.
Either way you can't view the webpage.
