I unblocked a few people on my list because I never see them around anymore. I really only block people if they block me first. There's been one or two jerks that I blocked because they sent me nasty messages, but that's pretty rare.

I usually block people who I have seen being rude or trolling or spamming on someone elses question.

Never. I normally only block people that spam my inbox, or I felt were reporter trolls.
