2002 Chrysler Voyager eC 2.4L I4; transmission not leaving 1st gear?
This van has 99k miles on it and my wife was leaving to goto the store one day and got it out on the road and up to 35 mph and noticed it wasn't shifting out of first. I drove it around and confirmed that it indeed woundn't shift from first; I even got it up to 50 mph and it still wouldn't shift. Fluid level and condition still looks fine. What could be wrong?

From what you're saying it could be that the TCM has gone into a limp mode and since you mention the speed indicator is working that also means that the rpm gauge is probably working as well as all the other gauges. If the TCM goes into the limp mode it only gives you one forward gear and this is just for emergency to get you to a local repair shop. The problem is you normally couldn't get the vehicle to drive 50 in fact the rpms would be way over 3,000-3500 just going 30-35 mph and if you could drive it 50mph you must of been way way up past 3500-4000 or more rpms.
Normally if any problem in the transmission control circuit especially if electronic the main comp would detect an error and then put the TCM into limp mode causing the engine light dash indicator to light up.The problem is sometimes a limp mode exists but the ECM doesn't indicate that any error code is stored or no engine light is on but in fact there is a code/codes stored that can be pulled via a scanner.
Hope that helps and best of luck.By the way first things I'd want to check is the transmission fluid level, then it's quality and also look for any possible fluid leaks.Other from that check for any error codes, check the fuses and any related relays.
