Should I form an LLC to teach guitar or is a sole proprietorship the way to go?

You could go either way. However, a LLC is more to protect your personal assets from someone who sues you - they can only sue to the extent of the LLC's assets.
If you do not anticipate high risk of lawsuit in your guitar school, going with a Sole Proprietorship is a less expensive way, with far less reporting obligations.
But if you're really scared of being sued - then forming the LLC may give you peace of mind. Or simply have a good insurance policy that covers the school and school staff.
