Do you have any advice on how to stop snacking?
I eat way smaller portions when it comes to a meal size than I used to; and I do feel satisfied when I'm done with the meal by the way. However, I have no idea if my smaller meal size is what makes me want to snack more often. My first choice is always an apple since it's sweet and low in calories. But no, I continue to have cravings; so I go get a banana. Nope, still no good. Maybe a little bit of cereal with little milk? Again no. This bag of chips should do it; NOW I FEEL GUILTY!
How can I stop myself from having cravings to snack? I work out everyday except for Sundays but I don't consider my workouts as intense as most people. I use a calorie counter app on my phone to help me determine when and how much should I eat and it does work; I HAVE lost weight. But I don't want to gain it all back just because of my snacking.

Is it possible you're maintaining too large of a calorie deficit per day? How much weight are you trying to lose per week? If you burn 500 more calories per day than you consume, you'll lose 1 pound per week (3500 calories = 1 pound). Hunger will obviously be more intense if you try to maintain larger calorie deficits.
Perhaps try allowing yourself to have a taste of such snacks, but do not have a large quantity. Remind yourself that hunger itself is not a horrible sensation. It is, in fact, a sensation that comes and goes and is not itself that bad of a feeling. It is often times our psychological response to hunger that is the real problem (reinforced through habits ingrained over time).
I wish you good luck! My main advice would be to perhaps be more scientific in regards to how much of a calorie deficit you're shooting for.
