Anyone know this movie?

This Movie is about a Boy and a girl fallingl in love and he is a high school football player that finds out he has cancer.

He was afraid to tell her and the doctors told him he would never be able to play again but he wanted to play the last game and soon after this he passes and Im not sure the part after this But The girl goes back to the High School where he played and on the field it said I love you in Flowers he had planted for her.
