See if you can find find a nearby dental school, university generally, where they need patients for their students to work on.

Here in Chicago, the county hospitals pull teeth for free. Hopefully the county hospitals there do the same.

Good answer, try the closest dental school. And then it sounds like your friend needs to reassess their life. no job no home? Time to get their act together, it's not the hospital's job to pull teeth, but it is your friend's responsibility to take care of themselves. No pity party.
