They are called favicons and there are millions of generators for them - http://www.google.com/search?q=favicon

Upload the file to your site and in the head section put the path to the file...

Put this in the <head> section of your page: <link href="icon.png" rel="shortcut icon">, where "icon.png" is an image that you can make either with GIMP, MS Paint or with Photoshop.
