I need help coding the Table of Contents for Kindel e-book?

My Table of contents works and you can click to the different chapters with no problem. But, when you are in the middle of the book, the Kindel button to return to the Table of Contents doesn't work. It says that it isn't available

    Contact Amazon and tell them you have a defective ebook. I assume you bought it from them. If you didn't get it from Amazon, oh well, you are stuck.
