Why do I continually have to put coolant/water in my radiator?

I have a 2002 Chrysler T&C. I replaced the upper radiator hose. My van doesn't overheat at all and I there are no leaks but I still have to keep an eye on my coolant. I'm hoping someone could tell me what I should check to stop this as I know it shouldn't be happening. I have noticed that when I have tried to run my AC it doesn't get cold either. I don't run the AC because of this problem as I don't want to do any damage to my van and I don't go much in it. I'm at a loss for what to do.

Update:

OK I don't have any white smoke coming from my tailpipe. I'm gonna replace my radiator cap today and see if that is it before I go do anything else. The engine light don't come on and it never over-heats so it's puzzling at the lest at the moment. Thanks to all who have given me things to consider checking or doing.

    Here is what you do first thing in the morning before you even start the van fill it with water and tighten the cap and drive it to auto zone go inside and ask one of the guys to please help you out and if they can pressure test your cooling system that you have a leak and cant locate it,and they will check it for ya for free. Mystery solved.,,,,,,,,,,

    First of all, your AC probably just needs a charge. Your coolant could be leaking from the rear heater or inside the engine through a bad head gasket. If you suspect a bad gasket, go to the auto parts store and get the leak sealer. I have used that with several cars and it works. Always buy the best stuff and it will cost you around $30.00.     If you HAVE to keep adding coolant, you have a leak. Denying it is futile, it's the ONLY place coolant will go.

    The leak can be external and evaporating, it could be internal, block or head gasket, or it could be the radiator cap.

    The ONLY thing you need to know, have to add coolant, have a leak.

    Coolant and AC are two different systems.

    sounds like you might have a head gasket leaking, if its the head gasket then it can be going into the oil or if its going out the exhaust you will see white smoke coming out when it is running
