Yes. On the application pay close attention to the question about Ethnicity.

No they would not, no college would. They expect you to have a good GPA and be able to handle extra curriculum's. And if you don't have a good GPA they would expect you to drop outside activities and concentrate on bring your grades up.
being good at extra curriculars is not going to get you through college level classes.

No, then everyone would just join clubs instead of studying. You can always take the exam again if you screwed up. Unless your school captain or sonething , they might offer u an interview.

Unless you are getting a scholarship (like sports) for your extra curriculars, they matter nothing to admissions.
