I need to frame 10 questions for the employees of any bank .And the topic is Job Satisfaction of employees ! I want questions which ask for the opinion of the employees.

I think there would have to be questions asking suggestions for work subsidized stress relief workshops, eg exercise classes, pilates, massage, counselor once per month
People are always suspicious about answering job satisfaction questionnaires. They have to be anonymous.
