Most teens enjoy slowly torturing a betta to death. When it's done they buy another. A few change their minds halfway trough. Anyone not doing water changes for a whole month definitely belongs in the torturing to death category. So, well done, it's nearly dead!!
Keeping a betta properly.
1. get a decent sized tank with heater and filter. 5 gallon at the very least. Despite the fact that they are kept in cups in the shop doesn't mean they don't like some room to swim. A 5 gallon or better will also provide some room for maintenance mistakes. Filters, look for something mild current, you don't need a massive filter. Sponge filters would work.
Also get a bottle of a good water conditioner that handles chlorine and chloramine. No untreated water should ever go into that tank. Salt is also unnecessary, it does not do anything for a freshwater fish.
2. Either cycle it properly (see below) or get a bottle of Tetra Safestart and used that together with some flakes for a week. Do a 60% water change after that before adding fish.
3. Bettas really like some cover. Real plants, silk fake plants if you must. Some like a cave. Don't use plastic plants they can cut their fins on it.
4. Feed as much as a betta eats in 30 seconds twice a day if they're still growing. Once a day and 60-90 seconds if they're not. Use a good staple like this http://www.hikariusa.com/bettabiogold/
An adult betta can fast one day a week without problems and will love the occasional treat of thawed bloodworm (3-4 at a time)
5. Bettas do not get "lonely" but if you like you can introduce a few small ornamental snails or, in the bigger tanks, a few shrimp.
6. Do NOT EVER EVER EVER replace all the filter media at once. You lose your beneficial bacteria and reset the tank to new.
7. The only time a betta will tolerate another betta is when they are ready to mate. At all other times a male betta will kill any other betta. It is possible to set up a sorority with a few females but you need a 20 gallon at least with plenty of plant cover.

Most people with betas have this problem. Just because betas can survive in almost no water doesn't mean they should. Betas are able to do that, because in their natural environment, the lakes sometimes dry up. Betas will get depressed faster than almost any other fish if they are not in the right size tank. A few gallons with a filter (you can buy at Walmart for like 20-30 bucks) You need to do this fast.

Male Betta's strive against. you are able to keep many lady Betta's with one male in a tank if it has places for the female to cover at the same time as they don't look to attend to to reproduce. The male makes a bubble nest on the floor of the water (frequently in a nook). Then the male and lady "dance" The male wraps himself around the female and squeezes the female until eventually she releases eggs. He then sprays the with his sperm, sucks them into his mouth and blows them into the bubble nest the position the stay until eventually they hatch. at the same time as choosing a woman fore breeding she might want to be a minimum of three/4 the body length of the male (do no longer count number fins). She might want to be crammed with eggs (look the female head on (into the Mouth) if she isn't rounded she isn't waiting to reproduce and the male would kill her. like maximum fish that lay eggs they are going to be very aggressive to different fish (chase the different fringe of tank or attempt to kill them). also If the pair is frighten after the eggs are laid they might eat them.

Sorry to tell you he is nearly dying! Is he in a 5 gallon tank or higher? Do you test your ammonia level or Nitrite etc. Do you feed him properly, Betta fish love blood worms! there food helps there color, I hope you help your betta fish!

What is different since he's changed?? Water change? Temperature? Filter? there has to be a reason. They just don't get sick for no reason.
