How do you create this camera lens flare?

I saw this on pinterest and tried it... Needless to say it didn't work. What am I doing wrong? Can anyone help me?

    Set your camera to manual AF mode and throw the subject lights out of focus. If you are using a mask the lights will take on the shape of the mask.

    It's called bokeh and the link you gave us shows you how to do it. Search for "custom bokeh" to find out where to buy those cutout things or how to make them and use them.
