I need some serious Help?
While I was playing at soccer practice my mind suddenly went blank.. this happens very rarely. I know im there and I can hear my surroundings but I just feel like I'm not there. When I got home I couldn't close my eyes, I mean I could if I tried but they would open by themselves slowly.. also I feel very wide awake..

Have you suffered any blows to the head during practice or games? Have you been diagnosed with concussions? When you can hear your surroundings, do they sound clear or muffled? Do you feel disoriented? You could force your eyes shut but they would open automatically, but slowly. And you'd feel wired (wide awake)?
Have you ever had a CT or MRI of your head? Have you ever had your blood sugars checked? I would schedule an appt with your doctor (obviously, ask your parents); he may need to refer you to a neurologist. You might be suffering from something as "simple" as a migraine headache (not every one gets an actual "head" ache), to something a little more involved, which might take machinery (like a CT scan) to detect.
Don't sweat the small stuff; in all likelihood, it's not an "anything" thing that a neurologist can't treat.

Well, there is a reason for this. Either you have had some injury? Or you are having some
type of panic attack and are leaving your body. If it is from injury go get some homeopathic
Arnica 30 c pills and take 4 pills am and pm till the injury is cleared- Arnica is for falls,
injury, bruises etc and works even if the injury was years ago.
If it was some kind of panic, you need to figure out the trigger- some thought is
triggering it or triggered it the first time and you are having something similar to the first
time go on. When you get far out of your body due to this- getting a shiatsu or acupressure
massage will put you back right ( see the mayo clinic on these)

I've had that type of thing happen before. I never figured out what happened. My guess is that it must be stress. I recommend doing something that calms you down like listening to relaxing music or breathing deeply.
Think back to the triggers. What happens and when? Do strange things happen when you're under pressure? Take note and I'm sure you'll figure out what causes it. YOU are your best psychologist, since only YOU know all about you.
