Yes. You're correct.
They were born with a condition that's most easily described as a "mind-body mismatch"

We all start out as sexually undifferentiated embryos. Around 6 weeks, in genetically XY individuals, the presence of the SRY gene on the Y chromosome causes the genitals to develop as a penis and testicles, as opposed to a vagina and ovaries. At this point, the brain itself has not formed. It is only a small clump of neurons. By the 11th-12th week, the sex of the body is fully established as either male or female, but the brain in the XY fetus remains "female" until it is subjected to an androgen "wash" which masculinizes it, in essence making the brain match the body.
If this process goes wrong in some way, the person can be born with a body that is masculine, but a brain that is not, and the person will feel like a female despite having male genitals.
For Trans men, the process is essentially the opposite. At some point the brain is exposed to high levels of androgens when it should not be, making the person feel like a male despite having a female anatomy.
Brain gender is a well established medical fact. There are numerous research articles that can be read at pubmed.org confirming this.

Yes, Jake is very correct, with one additional thing to know:
Being transsexual is not a mental disorder, as many people believe.
