We all start out as sexually undifferentiated embryos. Around 6 weeks, in genetically XY individuals, the presence of the SRY gene on the Y chromosome causes the genitals to develop as a penis and testicles, as opposed to a vagina and ovaries. By the 11th-12th week, the sex of the body is fully established as either male or female, but the brain in the XY fetus remains "female" until it is subjected to an androgen "wash" which masculinizes it, in essence making the brain match the body.
