It may be known across most of the US, but only has locations in 5 states - Arizona, California, Nevada, Texas and Utah. I have lived in Nevada and California and know that one of the first place out of state visitors want to go to is In-n-Out. Similarly, people from the west coast often make immediate pilgrimages to White Castle or other regional eateries when going back east.
I believe that they will only open locations within a 1 day drive of their distribution centers. They have high standards for food quality and never freeze their meat or other products. They have their buns custom made as well, so that limits the area they will serve.
This is the same for many other restaurants, such as White Castle and even grocery stores like Trader Joe's.

" Known " internationally. Often discussed on late night talk shows by guests as the first place they want to eat while visiting states that have locations. Their keep it simple ideal and quality have made them VERY successful and popular. They don't try to re-invent the burger every year and advertise the hell out of it to get people to come in, as most all their competition does as normal business. The concept of serving a limited menu and making it right seems to be lost on most of their competition. { notice how most of the " six dollar " burgers are now just short of six dollars ? }

As good as In-N-Out is No its only known in California, Nevada, Arizona, and part of Texas (there expanding in Texas)
