Where on the 1099R does it show 10% was withheld?

I took an early withdrawal from my retirement plan. The plan withheld my federal taxes and the 10% penalty. Where on the 1099R does it show this 10% was withheld?

    Any withholding is listed in the federal income tax withheld box. The penalty is shown separately--though it will be on your tax return.

    The total amount of federal taxes withheld is shown in box 4. It does not separate money withheld because of regular federal taxes from money withheld because of the penalty. It just all gets added in to the withholding amount.
