This may sound silly but i'm being serious. I'll try and keep it quick. It depends on the person, but I would be understanding. It might be a little weird to notice it without anything having been said, but I'd understand its embarassing for you. In such a situation I'd be silent too, not wanting to embarass you further, but in a way it's a compliment you got so into it.
As far as real sex goes... I'm not a guy... So that is hard to answer for me. But if in the future you are unable to hold you can still please your girlfriend after your own orgasm in different ways and I'm sure she'd like it. I do hate it when a guy does his thing, drops on his back, says "sorry", and we're done, without further care for my needs. But if he proceeds (not necessarily oral) I find that even more intimate and sensitive of him ;)
