how would you describe this personality trait?

for describing my character "he has a strange personality where he is one person, but also sort of dual personality; sometimes he has a side to him that can be manipulative and cunning, and more or less good and isolated, and another side to him that is completely polar opposite: arrogant, evil, and very sadistic, and brutally honest, and he can switch between these extremes at any time and they are not split “consciousness” that split personality portrays"

    A Gemini someone with two completely opposite personalities, I have an aunt like that, you catch them at the wrong time and its like world war three.

    No, really!

    Actually, your first "side" doesn't quite fit together, I don't see cunning and manipulative and more or less good and isolated as being kindred traits.

    I can see an "intelligent loner, who shows a meek and timid exterior to the world, while harboring resentments against those who put him down, who has lurid violent daydreams about how he would avenge himself on his tormentors.

    (I've had a few of those thoughts myself)

    But the other side you describe would be those thoughts breaking out after he stewed until he lost control (kinda like all the school shooting guys who go on a killing spree after they have been bullied one too many times)

    Otherwise, I guess you could describe behavior like that as being "manic depressive" or Bi-Polar.

    (seriously, I've had a couple of ex-GF's who really were mentally ill and needed medication to even them out. And I had a friend who had similar women in his life, his female friend and her daughter, who were both nutjobs. They could be nice and sweet and then in a flash would physically assault you and say all sorts of horrible things .My GF's were a manic depressive and the last one was paranoid schizophrenic. His were probably manic depressive drug addict and the daughter was bipolar )

    Seems to me that your character is at heart cruel and arrogant and has sociopathic tendencies who hides behind a meek and gentle exterior because he knows at heart he is evil. and it only takes one thoughtless, offhand comment or action that can irritate him and cause the monster to emerge.

    so, yeah, I think a sociopath describes him pretty well. They are cunning and manipulative and often put on a kindly face in order to indulge his evil desires. How many serial killers are described by their neighbors as a nice guy who never caused any trouble. Then they find that he has murdered 30 women and made lampshades out of their skin or something. Ted Bundy, BTK killer....etc...
