What is the best way to approach a woman in a bar or club?

I have just got out of a long term relationship. I been goin out to different bars and clubs to meet woman. I feel so rusty when it comes to speaking to them, bad enough I was never smooth to begin with. I mostly say hi how are you my. I ask them there name and give them mines. I try to make the conversation about them. Yet I strike out every time. I am a decent looking guy I am a vegetarian so I smell good and I am not fat. What am I doing wrong, any tips and advise would be a great help. And I pefer to here from woman on how they like to be picked up
