Make A Form Registration Web Page?

I want to create a Web page for my site so that users Can Register themselves and that regeneration information comes to my email automatically

Requirements in Form

Name, Date of birth, gender, phone no. with verification code (like after entering phone no. sms sent and code entered) or email verification (either on or both)

No password or something

Selection Boxes, address

Some Fields mandatory and if not filled, error message,
