Is this grammatically correct?

Both are correct imo as long as you put "sometimes".
It's probably technically best to say, "my room-mates really piss me off sometimes" but I don't know anyone who'd honestly make the difference.
You could also say, "my room-mates can really piss me off" and it would have the same meaning I think. As long as you include "can" or "sometimes" it shows that it's not all the time.

" my room-mates really piss me off sometimes " is the correct way of saying it

"Sometimes" must be one word. The use of 'can' as you note is optional. It really does not matter whether you put 'can' there or not, but the meaning is altered by the use of 'can'.
