Is the above sentence correct for writing a cover letter. Is there a way I can shorten it for a short cover letter and make it read betteR?

Implemented two Individual Behaviour Management Plans: Self Designed Behaviour Management Plan and The School Therapist Behaviour Management Plan.

(Recently) I have implemented two Individual Behaviour Management Plans; one developed by myself and the other designed by the school therapist.
If it is applicable, you could introduce a time element eg 'Recently' and in my opinion the sentence reads better with the removal of the 2 'was's

"I have two implemented Individual Behaviour Management Plans ; one was made by (state the name), a school therapist and the other was made by me. (You should always put yourself last)
