How do we know who he/she is referring to?

If i wrote "Jacob helped Gary out of the car. He went inside and got a drink."

Who is "he" referring to? What are the rules to using he/she?

The way it's written, I would guess that it was Jacob who went inside, but it could be taken either way.

You could rewrite it -- "Jacob helped Gary out of the car, then went inside and got a drink." Jacob is the one who went in for a drink

Or "After being helped out of the by Jacob, Gary went inside and got a drink." Gary is the one who went inside.
