In Parting of ways everyone died except The Doctor, Rose, and Captain Jack, but that is all I can think of because they try to make sure at least somebody lives other that The Doctor and his companions

Bit Late I know but I have one:
The Fifth Doctor's (Peter Davison) final story: Caves of Androzani leaves everyone dead, including the Doctor before his regeneration.
It's also arguably the best story the show has ever produced.
