Should I be concerned? I know I can get forms from the library, but I've always received it in the mail, until now.

If you changed addresses last year, then the IRS might not know where you live.

you can down load a copy at www.irs.gov

if you want forms sent to you, you have to request them either on the previous year's tax return, by phone at 1 800 826 3676 or go to the web site

if you've moved & the IRS doesn't have your new address then that's why you haven't received it. (they don't forward those).

just go to the library or post office & pick up what you need or file online

the federal is a 1040X and probably a similarly named form for NY

They stopped mailing them a couple years ago.
