Cocky is a shortened form of 'cocksure' which means arrogant, over-confident or conceited. It`s origin has nothing to do with male genitalia. Hence it is not considered a rude word.

Public opinion. That's the simplest and most logical answer. Just because that word contains slang for a body part it's actual meaning isn't vulgar and is simply an adjective describing a facet of someone's personality. The actual word itself is vulgar slang for penis or phallus so it's currently seen as a bit too colorful. Pretty obvious isn't it?
