It looks like you just used the rows and columns wrong.
For instance, the first row should have "canto" under "yo", "cantas" under "tu", etc. The first row is the conjugation of "cantar". You wrote it in the first column.
Similarly, the second row is the conjugation of "preguntar". They've already put "pregunto" in the 2nd column, so you need to write "preguntar" under "infinitive".
So just flip your rows and columns around and you should be fine. Your answers look correct.
