Drills or saws with special carbides are generally needed, for small cuts you can use a demolition saw http://www.amazon.com/gp/product/B003K4FQ6G/ref=as... or a core drilling machine (like a big heavy duty hole saw) http://www.amazon.com/gp/product/B004UB1YGY/ref=as... , you can remove large pieces by stitching holes in a line to make the general shape you need.
But if you need a serious piece of concrete out you are best off calling a concrete cutting and coring contractor.
