I'm an Egyptian Medical school grad with fluent English in a level of a native speaker due to my SAT education
Can I teach Medicine in Japan or what are my options at least?

If you have a PhD, you might be able to get a job as a researcher.
If you can pass the medical licencing exam (only in Japanese), you could get a licence.
Otherwise, your options are basically zero.
Given that you are not a native English speaker, even teaching English would be difficult.
I think you have few options to be picked up by any company, if you don't speak Japanese.
[edit; for completeness - unless you can teach medicine in Japanese, I think no university would hire you.]

Who are you going to teach to ? Medical schools teach in Japanese. And that means you would need to speak, read & write Japanese at a medical level. The Japanese way of thinking is also very different than what you have learned in your own country. What you learned there may not be acceptable in Japan.
Sorry, I see no options in the medical field.
Why do you want to work in a country you know little about ?
Your best bet is to find work in your own country for a few years.
And FYI, I really doubt you can have native level speaking from SAT's. Not possible.

I think the best you can aim for is post-grad research, and hope for some good connections that let you stay in Japan.
There is a slight possibility that you could teach English for a direct-hire Board of Education. I know two non-native English speakers who have done it. They had excellent academic records, and studied English for more than 10 years.
I think you could put your skills to better use in other places in the world, though.

With no job history, you won't be able to teach anywhere.
Native English speaker means it's your first language.
Japan's medical services are pretty closed to outsiders. Not many non Japanese in it.

No. You can't teach medicine in Japan unless you finished medicine study in Japan.
