iPhones are still sold SIM locked. If you asked for the AT&T model then it would be the reason for getting one with the SIM in it already. Until earlier this year, the closest thing to an unlocked iPhone was the one being sold through T-Mobile which was sold unlocked but still included the T-Mobile SIM in it.
As mentioned, the fully unlocked, no SIM included, iPhone 6 started to be sold earlier this year.
So its not likely your iPhone 6 is unlocked but the easiest way to tell would be to insert another carrier's SIM & see if it picks up their network.

Unknown. A dead-simple way to tell is put a SIM card from a different carrier in it. If it works, it's unlocked.
Based on your info, it's likely not unlocked because you specifically asked for a phone tied to a carrier.

You can try another carrier sim in there and see if it works or you can get you IMEI number from settings - general - about and check your IMEI number here - http://iphoneimei.info/
If it comes back unknown then you will need to try putting in a different sim, however unknown usually means it is locked.
