Ford Ranger hard to start?

Update:

I have fuel pressure gauge, turn the key to ON first time and get 10 psi, second turn ON I get 50 psi,

third turn on I get 60 psi.

Is fuel pump shot?

    Hard to start could be cause by other defects too but since you've mentioned FUEL so I'm focusing on the fuel system.

    The pressure should be 60 to 65 Psi with key on, engine off. Seems to me you do have restriction in the system. I would replace the fuel filter and see if the problem is taken care off. The fuel filter is on the bed frame rail driver side. It's the same for both 3.0L and 4.0L engines.

    If not, you are out of luck because the fuel pressure is controlled by the fuel pressure regulator which is mounted to the fuel pump bracket within the fuel tank.

    The truck is not "Flex" fuel, is it? You may have to check the alcohol contain because it may be the cause for hard start beside pressure problem.

    When you turn the key on and the pressure stuck at 10 psi or anything that less than 60 psi, while still KOEO, use a rubber mallet and hit the bottom of the tank. If the pressure come up to par, i would replace both the regulator and the fuel pump due to the age.
