Every Game Lags On My PC!!!!?

Every Game Lags On My PC Like Assassin's Creed IV : Blackflag On The Lowest Setting And Lowest Resolution I Get Average 15FPS My Specs Are:

Graphics Card: AMD Radeon 5450

CPU: Intel i3

RAM: 4GB

And I Think That Graphics Card Is The Problem Is It???

So If It Is Which Is The Best Graphics Card To Run Modern Games Like Watch Dogs And GTA V On Good FPS

So Which Part Of The PC Lower The Lag

Which Specs Would Be Great If I Want To Play Next-Gen Games Like : Tom Clancy's Division Or Mad Max

Or Should i get A Console But I Am A PC Gamer So I Have Problem To Shoot So If Yes Then Which Console Next-gen??? PS4 OR Xbox One Or PS3 And Explain Me The System Of Disk Space Of Consoles If I Have 500GB And I Run Games Like Watch Dogs Or GTA V So I Will Have To Install The Game??? Or Just Put The Disk In And Play And If I Have To Install Then If My Space Is Not Enough I Will Have To Delete???

    That 5450 HD Graphics card is obviously the biggest problem

    At least GTX 750 / R7 260 is what you need, to play current games at low settings at lower resolutions at playable fps

    Also that i3 and 4 GB also aren't very great. Not sure which i3 you have, since 1st gen i3's (530) are somewhat weaker for current games

    The reason all the games lag is because your computer isn't very powerful.

    The main processor is slow. It doesn't have enough RAM, and the graphics cards isn't powerful enough.

    You need to buy a whole new computer. Something like:

    Processor: i5 or i7 if you can afford it.

    RAM: 8GB or 16GB if you can afford it.

    Graphics: nVidia GTX750 or higher if you can afford it.
